This prestigious nonprofit Skilled Nursing Facility in beautiful and coastal Bay Area in Northern California is actively seeking for a Director of Social Services. The Director of Social Worker will join the family of dedicated nurses and other health care professionals living out a mission to heal people in body, mind and spirit. The organization offers services ranging from long-term care/skilled nursing residents and non-residential care through short-term and rehabilitation services unit for patients who require specialized interdisciplinary rehabilitation to recover from surgery or an acute illness, and through acute geriatric psychiatry hospital. The Director of Social Services will be responsible for the overall organization and coordination of social service delivery to residents, patients, families, as well as supervision of social services staff. The Director of Social Work will have responsibility to include discharge planning, clinical supervision, behavioral management programs, program development, professional outreach, translation services and public benefits. The Director of Social Services will work closely with Administrator, Director of Nursing, department heads, clinical staff and social work team to meet the psychosocial needs of residents patients and families. The Director of Social Work will work collaboratively with Director of Admissions and physicians to coordinate admissions and ensure smooth process. The Director of Social Work will develop and maintain written policies and procedures for social service delivery that are compatible with the overall philosophy and purpose as well as compliant to applicable regulations. The Director of Social Services will determine/develop a staffing plan for the social service department that maximizes the effectiveness and efficiency of social service delivery. The Director of Social Work will interpret and ensure compliance with legislative requirements for residents and patient care while setting the agenda and leading the weekly social service staff meeting and overseeing transfers and coordinates room changes when needed. The Director of Social Services will be responsible for timely completions of MDS assessments as they pertain to social services and serve as a resource for social workers in this area. The Director of Social Services will work collaboratively with IDT to implement non-psychological interventions to address resident/patient behavior issues and pain management interventions. The Director of Social Services will participate in Palliative Care committee meeting, staff meetings and educational in-services and prepare monthly QAPI process/committee meeting, staff meetings and educational in-services as required. The Director of Social Work will coordinate with psychologists and psychiatrists to facilitate psychosocial care and provide psychological support to STARS patients as needed and function as the Outreach Coordinator to consumer groups, and to the social services' professional community. Prepare monthly QI audits and compiles data to present at monthly QI meeting. The Director of Social Services will need a current and valid licensure as a Licensed Clinical Social Worker (LCSW) with 2 year's supervisory and 2 year's geriatric social work experience in a skilled nursing facility, preferred. The Director of Social Worker will need to demonstrate ability to maintain confidentiality and communicate effectively with patients and their families, and at all levels of the organization.
