Roles & Responsibilities Assess patients in skilled nursing to determine mobility, strength, balance, and functional needs Develop and implement individualized treatment plans that promote independence Provide hands-on therapy, gait training, and therapeutic exercises with compassion Document progress clearly and timely in the facility’s clinical record system Collaborate with nurses, occupational therapists, speech therapists and physicians to deliver coordinated care Educate patients and families on home programs, safety strategies, and fall prevention Participate in discharge planning to optimize patient transitions and outcomes Maintain compliance with facility policies, regulatory requirements, and best practices Qualifications Current, unrestricted California Physical Therapy license or ability to obtain licensure quickly Graduation from an accredited physical therapy program (DPT preferred or equivalent) Current Basic Life Support (BLS) / CPR certification Experience in skilled nursing or post-acute care preferred Strong clinical reasoning, documentation skills, and a patient-centered approach Ability to pass background checks and meet facility onboarding requirements Education Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program Completion of required clinical internships and state licensure Knowledge & Skills Proficiency in gait training, balance retraining, transfers and therapeutic exercise Comfortable with patient handling techniques and adaptive equipment Experience with documenting in electronic medical records and managing caseloads Knowledge of skilled nursing regulations, Medicare documentation expectations, and rehab pathways Excellent communication skills for patient education and interdisciplinary collaboration Ability to prioritize, stay organized, and adapt to changing clinical needs Benefits Generous housing stipend and travel support to make your assignment comfortable Medical, Dental & Vision coverage so you can focus on what matters Licensure assistance and reimbursement to keep your credentials current Paid time off to recharge between shifts Travel reimbursement and support with logistics for a smooth start A supportive team that values YOU and your clinical judgment Professional development opportunities and continuing education support Referral bonus program to reward you for bringing great teammates Travel Perks: Explore San Rafael, CA San Rafael sits in beautiful Marin County—think scenic hikes and coastal escapes.
