primarily provides medical care and management to members served by
the Landmark program who request urgent visits to the IDT team
during daytime and/or swing hours (8 hour shifts). This position
also participates in new patient enrollment, post-discharge visits,
and gathering clinical information for medical necessity
determinations for service requests. Dedicated to improving
the health and well-being of members, this position collaborates
within longitudinal Landmark provider and/or the PCP as needed to
discuss immediate and ongoing patient care needs.

visit, promptly communicates with the longitudinal provider and/or
pod leader to discuss patient assessment and
receive/document/implement orders received.

EDUCATION AND EXPERIENCE

- Minimum of two
(2) years clinical experience; Emergency Room or ICU care
