heard, respected, and empowered. POSITION SUMMARY: DBT serves
Alameda County youth at three engagement points: 1) adolescents who
entered foster care at ages 14-17; 2) young adults aging out of
foster care; and 3) homeless youth/young adults with foster care
histories up to age 21. The DBT Coach assess youth needs, provide
skills coaching and intensive case management services, and
coordinate services in the community that are intended to address
outcomes in the four areas education/employment, housing,
social/emotional wellbeing, and permanent connections. The DBT
Coach will be intensively trained in the Dialectical Behavior
Therapy (DBT) model and facilitate weekly skills groups and
coaching for DBT participants. The DBT Coach builds relationships
and facilitates a team-based approach among all the organizations
and staff who are engaged in the youth’s support team. Successful
DBT Coach’s understand how to motivate, influence, and mentor youth
and young adults to achieve goals while launching adult lives from
foster care. This position is required to work onsite and in the
community Monday – Friday. Essential Duties and Responsibilities:
Include the following. Other duties may be assigned. Client
Services - Facilitates weekly DBT skills groups and provides DBT
coaching. - Provides direct support, coaching, mentoring and
intervention to increase youth self-sufficiency and goal attainment
through weekly case management meetings with youth. - Coordinates
youth support team of service providers, family, and other natural
supports to ensure youth have necessary resource and supports to
address needs. - Develops partnership strategy with each youth to
develop creative ways for youth to practice and strengthen their
independent living skills. - Provides clinical leadership to
multi-disciplinary teams consisting of youth, family and other
natural supporters, county case managers, and external partners to
ensure seamless and effective service delivery. - Organizes,
schedules, and facilitates youth and team meetings. - Provides
on-going assessment of youth well-being and develops plans to
ensure safety and deescalate crisis as needed. - Keeps key team
members informed and communicating. - Participates on a rotating
basis to provide on-call coverage including DBT on-call coaching. -
Provides crisis counseling as needed. - Perform related work as
required. ADMNISTRATIVE - Completes documentation including but not
limited to Care and Services plans, needs assessments, and progress
notes. - Ensures all needed paperwork and consents are completed
for each client at time of entry. - Participates in agency
committee as assigned. - Provides on-going orientation and training
to staff as assigned. Qualification and Requirements: - Master's
Degree in Social Welfare, Psychology, or equivalent required. -
Must be eligible to register with BBS as an ASW, MFTi, or LPCi and
working toward professional licensure. - A strong dedication to
cultural competency that creates conditions that remove barriers
and increase a sense of belonging. - License preferred. -
Experience working with transition age youth with complex
presentations including chronic trauma preferred. Experience does
not substitute for professional degree and certification. - Must be
comfortable working in community settings. - Excellent
communication skills, professional demeanor, sound judgment, and
strong organizational skills. - Must be able to pass a Community
Care Licensing background check. - Must have a car, a valid CA
driver’s license and auto insurance - Attention to detail – Ability
to work with high level of detail, documents and products are well
thought out and clear, database is organized and up to date. -
Computer skills Proficiency with Microsoft Office applications
including MS Word, Excel and PowerPoint, Project and Outlook.
Experience with database systems. - Availability for occasional
evening and weekend work and some travel as needed. BENEFITS: - 15
